A collision at a busy Ashland County intersection turned into a deadly five-vehicle crash Saturday evening after a teenage driver entered the path of an oncoming Jeep, setting off a chain of impacts that left one person dead and three others seriously injured. The crash happened at U.S. 30 and State Route 511 in Vermillion Township, Ohio, at approximately 7:13 p.m. on October 31.

The Ohio State Highway Patrol said a 17-year-old girl was driving a 2010 Hyundai Elantra southbound on State Route 511. She was approaching U.S. 30 with a 17-year-old male passenger when the Hyundai failed to stop at the posted stop sign. The car entered the intersection directly into the path of a westbound 2013 Jeep Wrangler driven by 25-year-old Maximus Davis of Jeromesville.

The resulting impact sent both vehicles across the median. They eventually came to rest disabled in the eastbound lanes of U.S. 30, leaving the wrecked vehicles in the path of approaching traffic. What followed added another serious collision to an already dangerous scene.

A 2006 Acura TL traveling eastbound then struck the Jeep in the left eastbound lane. The Acura, driven by 19-year-old James Cavera of Bowling Green, Ohio, subsequently left the south side of the roadway. Two additional vehicles also hit debris scattered across the road from the initial collision, bringing the number of vehicles involved to five.

The teenage girl behind the wheel of the Hyundai was pronounced dead at the scene. Her 17-year-old male passenger sustained serious injuries and was airlifted by Metro Life Flight to Akron City Hospital. Authorities had not released the identities of either teenager as of Sunday.

The other two drivers also suffered serious injuries. Davis was first taken by Mifflin Fire & EMS to OhioHealth Mansfield Hospital before being flown to Grant Medical Center in Columbus. Cavera was transported by Jeromesville Fire & EMS to OhioHealth Mansfield Hospital. Both men required hospital treatment following the collision.

Despite the severity of the crash, investigators confirmed that everyone involved was wearing a seat belt. The Ashland County Sheriff’s Office and emergency crews from Ashland, Mifflin, Jeromesville and Hayesville assisted at the scene as troopers worked to establish what happened.

The Ohio State Highway Patrol said the crash remained under investigation Sunday, November 1. The intersection is located in Vermillion Township, Ashland County, approximately 23 miles east of Shelby and 24 miles south of New London. Authorities had not publicly identified the teenage victims.
